Your questions
are welcome here.
A little clarity before your appointment can make getting started easier.
Before your visit
How do I ask about an appointment?
Call (208) 918-0411 to ask about current services, eligibility, and appointment availability.
Can I have a telehealth visit?
Telehealth is an option, depending on your needs and location. Confirm that the practice can see you in the state where you will be during the visit. Some concerns require in-person care.
How should I prepare for a video visit?
Choose a quiet, private space. Check your internet connection, camera, and microphone, and follow the practice’s connection instructions. Have your questions and medication list available for your appointment.
Where do in-person appointments take place?
Confirm the appointment location directly with the practice before traveling. PO Box 5521 is a mailing address only.
What does a visit cost? Do you accept insurance?
Please call to confirm visit fees, payment arrangements, and whether your insurance can be used before booking. This website does not quote prices or confirm insurance participation.
Will I receive a prescription?
A prescription is not guaranteed. Any treatment recommendation depends on a clinical evaluation and whether that treatment is appropriate for you.
How do I share records or health information?
Ask the practice for its approved secure process. This website does not accept medical records, attachments, or messages with personal health information.
Is the patient portal available here?
The patient login page is under construction. Contact the practice for current access instructions. There is no sign-in form on this website.
